Three years later, Bloodborne enemy seen in PSX 2014 footage discovered in-game

During the PlayStation Experience in 2014, FromSoft video game director, Hidetaka Miyazaki, took to the stage to reveal the first footage of Bloodborne's procedurally generated Chalice Dungeons. The event  revealed how Chalice Dungeons would work by showing off a bit of gameplay from one.

In this gameplay, we saw a flame creating undead giant with candles on its back. Since 2014, the enemy had not been reported to be encountered in the game. Luckily someone was dedicated enough to put the effort into finding it. 

The enemy was finally located in a Depth 4 Cursed and Defiled Root Chalice Dungeon with the glyph "pa6ssc6u".
